• Parent’s full name (who carries the dental insurance)
• Parent’s date of birth and mailing address
• Employer name
• Dental insurance company name
• Insurance ID or social security number

Understanding Your Coverage Your insurance company can best explain your specific benefits, including deductibles, yearly maximums, and coverage percentages for services like x-rays and sealants. They can also help you understand the difference between in-network and out-of-network coverage. (Please note: we are an out-of-network provider, though most insurance plans still provide benefits.)

Payment Options

• If your insurance pays us directly: You’ll only need to pay your portion at the time of service
• If your insurance pays you directly: Full payment is required at your visit
• No insurance or out-of-network restrictions: Full payment is required at your visit
